Application Process

To stay at Faith's Lodge, each couple or family must receive a referral from a member of their healthcare team or faith community, a mental health practitioner, or an approved support organization, such as the Ronald McDonald House.

For Professionals Making a Referral:

There are two ways to make a referral for a couple or family:

1. Call Faith's Lodge at 715-866-8200 to speak directly to a staff member.
2. Download the appropriate application and fill in the referral source assessment and contact information. Pass along the application to the interested couple or family, and ask them to complete the remainder and submit to Faith's Lodge.

If we have not had phone contact with you prior to receiving the application, a follow-up call will be placed to you once the application is received.

A $25 non-refundable application fee must accompany each guest application. The cost for first-time guests to stay at Faith's Lodge is $25 per night. The cost for returning guests is $75 per night. For Guests of Faith's Lodge:

Once your application is received and reviewed, a staff member will contact you to confirm the availability of your preferred date of stay or to discuss our waiting list process. Following this discussion, you will be sent a welcome packet with specific information about your stay.
